Clinical Care Strategies

Professional development of our rehab staff is ensured through the use of our Clinical Care Strategies. These strategies are divided into Skill Sets and Care Protocols.

Clinical Pathways
Clinical recovery periods have been identified for the most common diagnosis that we see in our facilities. A clinical pathway is an integral part of the patient’s care map that fits into the patient’s entire clinical recovery period.
Long-Term Care Patients
Long-term care programming is designed to meet the needs of all long-term residents as well as OBRA requirements. Programs for our long-term resident addresses patient needs in their own environment. Program development is tailored to the specific needs of the facility.
Documentation
Peoplefirst Rehab policies and procedures documentation and billing systems will be utilized. All therapists are held accountable to the Peoplefirst Rehabilitation clinical rehabilitation policy and procedures per the CCA.

Documentation will include information necessary to communicate patient need, rehabilitative services provided and progress made.
Quality Assurance
Peoplefirst Rehab provides audits of billing and documentation that ensure compliance with Medicare and Medicaid regulations. Audit results will be available to the facility’s coordinator to support quality assurance programs.
Point of Care (POC)
Peoplefirst Rehab’s proprietary, nationwide clinical information management system provides infrastructure for every major aspect of our data collection. Information transferred to a centralized database provides continuous analysis of our clinical results and is used in the successful management of our rehab programs. We offer custom-designed report packages to fit our customer needs.
Continuous Quality Improvements
Documentation audits are an ongoing focus in each of our rehab departments. Results of the clinical audit process are a part of the Total Quality Improvement process. Peoplefirst Rehab’s audit tool is automated in the POC system and is specific to Peoplefirst Rehab’s documentation system.
Medicare Management
Quality care is provided for our Medicare A patients which incorporates PPS regulations as well as MDS requirements. Included in the training is communication between facility and rehab staff.
Claims Management
Peoplefirst Rehabilitation’s Client Services Department has developed a computerized Denials Tracking Log. This database enables Peoplefirst Rehabilitation to track Medicare denials by facility, discipline and reason for the denial. This proactive approach identifies intermediary trends and the need for staff education and training.
Dementia Management Program
Peoplefirst Rehabilitation’s Dementia Management Program is designed to focus on the ABILITY - not inability - of the patient, and adapting the environment to enable each individual to function at the highest level possible. We utilize assessment tools and cognitive compensatory strategies to aid in achieving results.
